1. Consent to Contact
By completing and submitting this form, you consent to Plumfield Dental Practice contacting you by telephone, email, SMS, or other reasonable means regarding your enquiry and the free practice visit offer. You may withdraw your consent at any time by contacting the practice.
2. Eligibility and Triage
Submission of this form does not guarantee a free practice visit. All enquiries will be reviewed and triaged by the practice to determine eligibility for the offer. The practice reserves the right to decline any request at its sole discretion.
3. Nature of the Visit
The free practice visit is intended to provide prospective patients with an opportunity to learn more about the practice, meet members of the team, and discuss available services.
This visit:
Is not a dental examination or “check-up”.
Does not include prescriptions, radiographs (X-rays), or treatment.
Is not intended to assess oral health, identify dental disease, or provide reassurance that no dental problems are present.
Should not be relied upon as evidence of dental fitness or oral health status.
Is not suitable for urgent or emergency dental concerns.
Does not establish a dentist-patient relationship.
4. Member of Team Conducting the Visit
The visit may be conducted by any member of the practice team deemed appropriate by the practice. Attendance at the visit does not automatically include time with a dentist, therapist, hygienist, or any specific clinician.
5. Duration of the Visit
The visit will be no longer than 15 minutes. The actual duration may be shorter depending on the nature of the discussion and the needs of the prospective patient.
6. Pre-Acceptance Paperwork
Where requested, all pre-acceptance paperwork must be completed accurately and returned before the scheduled visit. Failure to complete the required paperwork may result in the visit being cancelled or postponed at the practice's discretion.
7. Changes and Cancellation
The practice reserves the right to amend, reschedule, withdraw, or cancel the visit or offer at any time without prior notice.
As this appointment is provided free of charge, patients are requested to provide as much notice as possible if they need to cancel or rearrange. Failure to attend a scheduled visit without notice, or cancellation at short notice (less than 24 hours before the appointment), may result in the practice declining to offer a further free practice visit or similar promotional appointment in the future.
8. Registration with the Practice
Attendance at a free practice visit does not oblige the individual to register with the practice, nor does it oblige the practice to accept an individual as a patient.
9. Data Protection
Any personal information provided through this form will be processed in accordance with the practice's Privacy Policy and applicable data protection legislation.
